4/23/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, JaCarei entered the session in an excited and eager demeanor, evidenced by his need to explain to the QMHP-C that he had been following his routine. Mother engaged QMHP-C in a discussion about his behaviors in the home setting and mother reports he just have issues with expressing his feelings. 2. Behavior: JaCarei displayed hyperactive behaviors for roaming around the room not paying attention, daily 2-4 times throughout the day, moderate to severe intensity. QMHP-C began the session by inquiring about JaCarei's emotional well-being. QMHP-C assisted JaCarei on brainstorming all the positive effects of him following directions and completing his responsibilities by meeting the QMHP-C on paper. QMHP-C assisted JaCarei in explaining why he always states to the QMHP-C that he was fine and well behaved at home and his parents explained how bad his behavior had been. QMHP-C reviewed JaCareiâs behavior at home and if he had experienced any symptoms at home. QMHP-C assisted JaCarei in expressing how he was acting and behaving at home. QMHP-C used active listening and supportive statements to encourage JaCarei, as he shared his thoughts on his actions and progress. QMHP-C used positive feedback and compliments to support JaCarei and urged him to get on task. QMHP-C assisted and brainstormed more thoroughly instances were JaCarei was rude and had misbehaved such as yelling at his mother and sister again. 2. Intervention: QMHP-C discussed with JaCarei some of the triggers he feels make him upset and angry like meeting QMHP-C and not being able to spend time with his older sister. QMHP-C elaborated for JaCarei how he feels when he first gets upset and begins to recognize he is mad or hurt. QMHP-C educated JaCarei on using some counting down from ten to calm down before reacting to his feeling when upset. QMHP-C used active listening and supportive statements to support JaCarei while expressing himself and gave some suggestions to him. QMHP-C used suggestions to assist JaCarei with understanding what to call his symptoms and how to work through them. 1.Response: JaCarei was receptive to QMHP-C's suggestions for today's session. JaCarei listened to encouragement and positive feedback from the QMHP-C. JaCarei discussed some of the issues he has with listening to his parents directions and not being able to do whatever he enjoys doing before getting angry for being redirected, such as meeting with the QMHP-C. 2. Response: JaCarei was able to participate and discuss his triggers after his parents demanded that he join QMHP-C in the session, he expressed that he enjoyed seeing QMHP-C and had followed directions, evidenced by his âdo you think we can play magic cards after we do the activity. JaCarei was able to draw and write his interpretation of how his parents would feel if he listened and spoke to them more respectfully. JaCarei was able to draw his family with smiling faces as he explained that that is how they react when he listens and follows directions. JaCarei was able to write down the pros and cons of listening and being respectful.
